oTo-Tour Driver License

Pre-Application (Step 3 of the oTo licensing process). Modeled on the Minnesota Class D driver's license process and REAL ID document standards, adapted for autonomous Segway / oTo-Tour operation.

⚠️ TEST MODE — no real money is charged and licenses are not honored on-site (yet).

Application Type — choose your license class

Both classes cost the same $40, and both get the same $20 back when 1,000 people sign up. Your class is about how much information you share, not how much you pay — and it sets your discount on all future tours.

The form adapts to your class: pick Class D and you'll only see the essential questions; pick Class R and the full-profile questions appear below.

Class upgrades are retroactive. Not sure? Start with Class D — share the full-profile information later and you move to the Class R refund, even after the payout (we top up the difference). You never lose money by deciding slowly.

A. Applicant Identity

🔒 We do not ask for your Social Security Number — not optional, not attested. An oTo license doesn't need it, so we don't store it.

B. Residency

📎 Document upload (two proofs of residency) — coming soon.

C. Physical Description (voluntary)

Every question in this section is optional — answer only what you are comfortable sharing. Skipping them does not affect your application.

D. Contact & Emergency
E. Medical & Fitness-to-Operate

Questions marked * are required — they are about your ability to safely operate an oTo unit, not about who you are.

F. Driving History & Legal Attestations
F2. Applicant Statements (per MN form PS33100)

All applicants must answer these statements.

All driver's license and instruction permit applicants:

Provisional licensed drivers only:

Commercial (guide/operator, oTo-REAL) applicants only:

G. oTo Operational Preferences
H. Donor & Opt-In Preferences
H2. Race & Ethnicity (voluntary)

Per MN Statute 171.06 you may disclose race and ethnicity. This is optional, is never printed on the card, and leaving it blank does not affect your application.

Automatic voter registration notice: on the real MN form, providing documentation demonstrating U.S. citizenship sends your data to the Office of the Secretary of State for voter registration, with a mailed opt-out. Shown here for process fidelity only — this mock sends nothing anywhere.

Sections I (photo, signature, biometrics) are captured at the on-site kiosk. Your Application ID and QR code are generated after payment.
